In Sydney, the least frequent route would probably be Route 388 operating from Paddington (Lawson St at Vialoux Ave) to Bondi Junction. It operates once every weekday in just one direction.
I'm surprised that the 388 and 342 (one of Sydney's shortest routes) survived privatisation as the campaign against it suggested privatisation meant longer bus routes. After all the 388 follows the much more frequent route 389 for nearly its whole journey.

Transport for NSW controls the routes which are operated in the various regions and their frequencies - so privatisation of the operator concerned should not make any difference in that respect..

388 was just a short running 389 until after the 389/379 split. Why they gave it another route number: ¯\_(ツ)_/¯

Route 281 on the Central Coast, NSW operates two trips Fridays only.

Route 1201R from Swan Reach to Renmark in South Australia’s Riverland only operates once every 8 weeks - that will be hard to best (or worst).

I don’t think it even deserves the title of “service”. I guess Swan Reach has population of 300ish, and connects to Berri and Renmark (both with about 4000), so it’s unlikely to ever attract much.
A weekly service as far as Berri is provided, but still not much use. If someone was really keen they might consider combining the several Swan Reach services (on different days, to different regional centres) with the Karoonda services, which seem to link similar centres. But unlikely to happen in the near-mid future unfortunately.
