This is a direct result of a Liberal government nobbling the station by taking half of Elizabeth St subway for its Southbank project. In the process, the subway to pfm 10 access was sealed off.
Next, pfm 11 was closed to build a long riverside bar, and this new entry (mismatched as usual fossilises that).

A new entrance to Flinders Street Station from Southbank is being built to ease pedestrian congestion at Melbourne's busiest railway station.
Meanwhile, 240 new CCTV cameras and a new intruder alarm system will be installed inside the City Loop to reduce disruptions caused by trespassers.

"That will deliver immediate notification to the operator if there is anyone trespassing on the tracks, so this is about improving public safety and also improving the reliability of our train system," Acting Premier James Merlino said on Monday.
The Southbank entrance will connect directly to platform 10. Photo: Supplied There were about 60 intrusions into the City Loop two years ago, causing major delays to services as police tried to locate the trespassers, Public Transport Victoria chief executive Jeroen Weimar said.
"We've already seen a reduction of around 50 per cent in the last year and we expect to see that number go down significantly as a result of this investment," he said.
The new Southbank entrance will connect directly to platform 10, which services the busy Werribee/Williamstown line.
The entrance will open in April. Photo: Supplied It is expected this will relieve pressure on the congested Elizabeth Street pedestrian underpass.
The entrance, which will open in April, is being built as part of the Andrews government's $100 million Flinders Street Station upgrade.
The Elizabeth Street underpass will also be renewed as part of the upgrade.

There is still a ramp between platform 10 & the Elizabeth St subway.

You are right in the sense that they should have widened the subway when it gained the dual purpose of a link to Southbank, rather than the current metal fence down the middle. This could have also addressed the lack of DDA access at the Elizabeth St end of the subway (DDA access is available to access Southbank).

The new access to Platform 10 is good, but there are still no plans to widen the subway - seemingly only to spruce it up. Had the commercial development along the river bank not been allowed, the Degraves Street subway could have been extended to give access to Southbank.

Elizabeth St subway can't be widened along the western side: it is alongside the Elizabeth St barrel drain, feeding the underground creek to Yarra River.
The subway to pfm 10 stairwell, which is sealed off, climbed over the descending drain.
The ramp doesn't really feed pfm 10, it points at pfm 12.
This scheme, as with the long bar, blocks reactivation of pfm 11. The useless PTV 'we don't need that platform'. Now build us a megabillion tunnel because we can't cope.

One neat scheme would be all Newport to the Sandringham line via 10/12 eastbound, and 13/11 westbound: nearly a real metro.
Williamstown - stopping all - Flinders St - limited express - Sandringham, and Werribee - limited express - Flinders St - stopping all - Brighton Beach.
6 tph each, with three more paths per hour available.
